Please specify your choice of dental
prosthesis line: you must
write on the order form or on your prescription which prosthesis
line is to be used, tariff A or
B
When this is not specified
by the practitioner, our laboratory will automatically carry
out the work according to the green line, tariff A

Provide us with a maximum of additional information
in the box marked "observations" on our order form |

Do not write anything in the upper right corner
of the order form |
|
| |
 |
You must place your professional stamp, your patient's
identification (name or your practice's internal identification
code) and your signature in the lower left part of the top copy |

 |
Keep the third copy and send us the other two
copies |
|
| |
 |
The fourth page of your order form (shuttle):
You must keep this copy in the case of prosthesis work which
requires a preliminary try-in. You must attach this copy to
your work during the finishing stages |

In the case of a telephone call concerning an
order: please provide us with the order number which is shown
in the upper right corner of our form |
